Office Hours

Our office staff is available to assist you Monday - Thursday 8:30 AM - 3:30 PM, and Friday 8:30 AM - Noon.
Surgical procedures are typically scheduled on Tuesday, Thursday afternoons and Friday afternoons.

Office Holidays

Our office will be closed on the following holidays in 2019.  Please note that a physician will be on call for emergency purposes.  Medication will not be refilled on holidays as this is not considered an emergency.

Thanksgiving - Thursday, November 28 - Friday, November 29 
Christmas Holiday - Tuesday, December 24th - Friday, December 27th
New Year Holiday - Tuesday, December 31st - Wednesday, January 1st

Appointments

We understand that you have many choices when it comes to choosing a pain specialist, so we have made requesting an appointment a simple process via our website. If you are a new patient, please contact our office at 832.437.3688 to schedule your new patient appointment or use the Request an Appointment form to contact us. Please note that we may require a referral from your primary care physician or surgeon.  Existing patients can request appointments through the patient portal. Click here for the patient portal.

Cancellation Policy

If you need to change or cancel an appointment, please do so 24 hours prior to your scheduled appointment. Our office policy requires 24-hours notice for all cancellations. Unless the office is properly notified, you will be billed $50.00 for the missed appointment. Please note that insurance companies will not reimburse you for a missed appointment.

Medication Refill Request Policy

Refill requests can be made through the Patient Portal.  You must allow 48 hours for us to complete the refill.  There will be no refills of medication after hours or on weekends & holidays

Note that we are currently considered out-of-network for all BlueCross BlueShield and Aetna insurance products.  Our office will provide a complimentary benefits check and submit your claim for payment even though we are not a participating provider.  Please call your insurance carrier to confirm your understanding of your benefits.  Insurance is ultimately a contract between you and your insurance company, and not with us.
 
We take our relationship with patients very seriously.  Because of this, we carefully choose the insurance carriers with whom we participate.  We will not allow an insurance company to interfere in the doctor/patient relationship.
